Hey,
I totally and completely know where you are comming from. I have taken Piano for 3 years now. I go to a Christian school and every week the chapel leader asks me to play the next week. I hate saying no, but I can't exactly just sit down and follow the guitar (which is what she expects me to do. One thing I will suggest though is that you sit down and practice ear playing different worship songs that you know. Also practice different chords, like G chords and C chords. Get familiar with the ear playing the piano, that way, when the guitarist plays, you will be able to pick out a chord from the keyboard.

Good advice. Any pianist/keyboardist in any style should be able to sit down and play at the very least any major or minor chord that they are instructed to play. From there, things like sevenths, suspensions, and added notes are easy to figure out.

It's highly unlikely that the music leader will expect you to be able to pick out what chords the guitarist is playing by ear alone. I would guess that the guitarist is following a chord sheet and that you would be able to have access to the same sheet. Chords are the same for guitar and for piano, so you can just play what is written on the sheet.

If it is what to do with the chords that is bothering you, I recommend reading through this thread, as well as all the others I have linked to.
