I just listened to this a half dozen times and attempted to translate piano to
acoustic. This is just my translation. I think its pretty but a little
difficult in some places but hopefully these chords will help you.

For the second time through the chorus I like to keep the third fret of the
last two strings covered throughout - one, because it is really easy to play
all the chords, and two, I think it sounds beautiful that way. I use this on
all but the D chord which an optional Dsus4 – D lick could be added just to
make it sound prettier.
There is a wind instrument playing before the second chorus and is a little
hard to translate to acoustic (translating piano is challenging as well) so I
just did my best to follow with what I thought would sound the best.
Note the key changes from the first chorus to the second and from the second
chorus to the second verse. And the end may sound better a’capela than trying
to accompany with guitar. Personally, I like singing it more than playing it.
And if ANYONE knows an easier way to play the Bb I have listed, me and my
fingers would be much obliged to hear from you. Here you go – You Raise Me Up:

Actually, an easier way to play Bb without barring is 688700 or just replace the 00 with XX. Um, the second pre chorus and second chorus is the music between the first chorus and second chorus. I'm pretty sure that's what they play. Don't quote me on it, though. See for yourselves.
I like to use the e-shaped bar chord for Bb: 688766. This chord works well after an A-shaped Dm chord. As for the easiest way to play Bb, you just have to do it over and over again, until you have the finger strength to play a full bar chord. I found that playing a Bb XX0221 is easy, but develops bad habits. When I was able to play a bar chord, I had to re-learn several chords, and play them the right way. Hope this helps.
Great tab! I had to change some of it to make it more comfortable for my voice, but very good! It really sucks that they don't allow to you put the lyrics on here. :(
