Those chords are way awsome. But I noticed that on the outro (End) you kept the chords the same as the verses. I'm not 100% sure, but I think the outro is something like-

//D A A# G// D A

[Repeat 4 times]
A# G D A
The noise has broken my defense, let me embrace salvation.
A# G D A
Your voice has broken my defense, let me embrace salvation.
D Bb C D
let me embrace, let me embrace salvation.

-The A# might be an A# minor, you can play around with that.
Also the chorus I think goes something like this-

D G B
You looked into my life and never stopped, and you're thinking all mu thoughts
D A
are so simple, but so beautiful.


-And remember the chorus is played in a different timing then the rest of the song.

Bm is a 5 string minor barre chord at fret 2
1 finger goes oer5 string and 1 string,
3nd finger is on string 4 at fret 4
4 finger is on string 3 on fret 4
2 finger is on string 2 on fret 3
this is a B minor barre chord with its root on string 5
